First Contact Physiotherapists (FCP) Jack, Chris, Karl A first contact practitioner (FCP) is an experienced physiotherapist who has the advanced skills necessary to assess, diagnose and recommend appropriate treatment or referral for musculoskeletal (MSK) problems on a patient's first contact with healthcare services e.g. when they visit the surgery. The FCP's can refer for X-ray & MRI scans, as well as arranging joint injections. |

Care Co-ordinator An integral part of the multi-disciplinary team, a care coordinator will work closely with the patient to initially assess their needs, in order to develop and deliver personalised and often complex care plans. As well as connecting the patient with the right people within the practice, they may also help with bringing together different specialists and community service providers to ensure the care delivered is appropriate and effective. |
